After an attack of chicken pox, VZV retreats to nerve cells within the ganglion or spinal cord, where it lies dormant for several months to decades. Aging, stress, or disease will cause VZV to reactivate and reproduce, at which point it is known as herpes-zoster. Once activated, the zoster virus travels along the nerve path to the skin’s surface and causes shingles. Treatment involves antiviral drugs such as acyclovir, or prodrugs such as famciclovir or valacyclovir. Maximum efficacy of antiviral drugs occurs if treatment is commenced within 72 hours of symptoms. This case underscores the importance of performing a thorough physical exam and the importance of avoiding premature closure of the differential diagnosis.
